Phase reversal occurs frequently in industrial power grids following transformer maintenance or storm repairs. When incoming 3-phase lines swap from R-Y-B to R-B-Y, 3-phase electric motors run in reverse, causing catastrophic mechanical destruction in elevators, air compressors, and water pumps.
